Medical Documentation: Please send appropriate medical information. Our neuropsychologists provide integrated reports that require medical information in addition to our cognitive testing. Appropriate medical documentation could include:
- Patient summary (medications, medical diagnoses)
- Primary Care Notes
- Documentation on major illnesses or injuries (e.g. TBI, car accidents, COVID with complications,)
- Major surgery summaries
- All previous psychological or neuropsychological assessments
- All neurology notes
- Brain imaging impressions
- Psychotherapy notes
- Psychiatry Notes
